This section of the manual contains the instructions for entering OMC data into the appropriate PGA screens.

Disclaimer	Conditional	A code indicating the reason data isn’t being provided.
OK/Cancel	Buttons	Click OK to save the PGA Information and move on to the next screen. Cancel will cancel and go back to the PGA screen without saving.

Field/Button	Status	Description
Electronic Image Submitted	Conditional	Optionally required if image of label, letter, or additional information is provided for the Entry. Such information may be provided in an attempt to prevent a Documents Required request later in the process.
Item Type	Mandatory	Defaulted to P for Product.

Field/Button	Status	Description
Add/Edit/Del	Button	Click Add or Edit to open the Documents screen. Click Del to delete a line.
Document Identifier	Mandatory	Must be “924”.
Conformance Declaration	Mandatory	Use this to indicate the manner of harvest. For Box 7, the valid box numbers from form DS- 2031 are “7A1”, “7A2”, “7A4” or “7B”. For Box 8, the only valid box number from form DS-2031 is “8”. Note: “7A3” is not a valid option at this time.
Declaration Code	Conditional	If Box 8 is provided, must always be “DS1”.
Role Code	Mandatory	Box 7 must always be “EX”. Box 8 must always be “RGO”.
Declaration Certification	Mandatory	Must always be “Y”.
Signature Date	Mandatory	Date of the signature in MMDDCCYY (month, day, century, year) format.

Source Type Code	Mandatory	Must be “HRV”.
Source Country	Mandatory	A two-letter country code that identifies where the harvest, growth, or catch took place. If the Conformance Declaration in PG22, at positions 13-17 = "7A2", then the 'Country Code' must = "AU" or "FR". If the Conformance Declaration in PG22, at positions 13-17 = "7A4", then the 'Country Code' must = "AU” or “ES". If the Conformance Declaration in PG22, at positions 13-17 = "7B" , then the 'Country Code' must = "AR", "BS", "BE", "BZ", "CA", "CL", "CN", "CO", "CR", "DK", "DO", "EC", "SV", "FJ", "FI", "GA", "DE", "GT", "GY", "HN", "HK", "IS", "IE", "JM", "MX", "NL", "NZ", "NI", "NG", "NO", "OM", "PK", "PA", "PE", "RU", "LK", "SR", "SE", "GB", "UY" or "VE"
Processing Start Date	Mandatory	The date the product leaves the harvesting nation.

Add/Edit/Del	Button	Click Add or Edit to open the Product Entities screen. Click Del to delete a line.
Entity Role Codes	Mandatory	Identifies the role of the entity. For OMC, must be one of the following: “AQF”, “EX”, or “RGO”.
Entity ID Type	Conditional	Identifies the code being used to describe the entity, for example: DUNS, IRS number, FDA Facility Code, Manufacturer ID.
Entity Number	Conditional	Identifier for the Entity.
Name	Conditional	Name of the entity.
Address 1	Conditional	Address Info MANDATORY when entering Importer, Certifying Individual, Storage Location and Owner
City	Conditional	City of the entity, mandatory if name is entered.
State	Conditional	State of the entity, mandatory if name is entered.
Country	Conditional	Country of the entity, mandatory if name is entered.
Address 2	Conditional	Address 2 of the entity.
Apartment/Suite Number	Conditional	Apartment/Suite Number of the entity.
Postal Code	Conditional	Postal code of the entity, mandatory if name is entered.
Add/Edit/Del	Button	Click Add or Edit to open the Individual Entities screen. Click Del to delete a line.

Name	Conditional	Name of the individual at the company. MANDATORY for all Roles
Individual Qualifier	Conditional	Individual Qualifier MUST match the Entity Role Code.
Phone Number	Conditional	Phone number of the individual at the company.
Email	Conditional	Email of the individual at the company.
